Findings and Diagnosis
A 49-year-old male patient arrived at Indimasi with severe cervical and lower back pain radiating to both upper and lower limbs. The symptoms had rapidly progressed over two weeks, significantly affecting mobility, sleep quality, digestion, and daily functioning.
MRI findings revealed multiple spinal degenerative changes, nerve root compression, disc bulges, and bilateral facet arthropathy affecting both the cervical and lumbar spine. Alongside the physical symptoms, the patient also struggled with constipation, acidity, disturbed sleep, numbness, and mental fatigue caused by chronic pain.
The treatment approach at Indimasi focused not only on pain relief, but also on correcting digestive imbalance, reducing inflammation, improving spinal flexibility, calming the nervous system, and restoring emotional well-being through an integrated Ayurvedic healing program.
Day 1–2: Initial Assessment and Detoxification Phase
At admission, the patient reported severe throbbing pain in both the cervical and lumbar regions with a pain intensity of 6/10. Movement, exercise, and positional changes aggravated the pain considerably.
Clinical examination showed:
- Restricted neck and lower back movements
- Swelling and Grade 3 tenderness
- Positive Straight Leg Raise (SLR) test bilaterally
- Morning stiffness lasting approximately 10 minutes
- Mild gait disturbance due to pain
The initial treatment plan focused on reducing aggravated Vata, improving digestion, and preparing the body for deeper therapies.
Internal medicines were initiated with:
- Amapachanam (detoxification and digestion-enhancing therapy)
- Deepana (digestive and metabolic stimulation therapy)
- Vatanulomanam (bowel regulation and Vata-balancing therapy)
The patient was also started on a customised Kapha diet plan to reduce inflammation and digestive stress.
External therapies included:
- Udhwarthanam (herbal powder massage for stiffness reduction)
- Abhyangam (full-body Ayurvedic oil massage)
- Choorna Pinda Swedam (herbal bolus fomentation therapy)
During the first two days, the primary goal was to reduce muscular tightness, stimulate circulation, and improve digestive function.
Day 3–4: Early Pain Reduction and Improved Mobility
As therapies continued, the patient began noticing the first signs of improvement.
Pain intensity gradually reduced from 6/10 to 5/10. The severe throbbing sensation started becoming less intense, particularly in the cervical region.
Taila Snehapanam (internal oleation therapy with medicated oils) was initiated with carefully monitored increasing dosages starting from 15 ml.
Additional therapies included:
- Lepam (medicated herbal paste application)
- Pichu (localized oil retention therapy)
- Nasyam (nasal detox and neurological therapy)
Yoga and pranayama sessions were introduced twice daily to improve flexibility, breathing patterns, circulation, and nervous system relaxation.
The patient also started experiencing mild improvement in bowel movements, gas, and acidity symptoms.
Day 5–6: Transition from Throbbing Pain to Aching Pain
By the sixth day, the patient reported a major shift in pain quality.
The pain intensity reduced further to 4/10, and the severe throbbing pain gradually changed into a dull aching sensation. Morning stiffness and swelling also reduced noticeably.
Specialised local spinal therapies were intensified:
- Greeva Vasti (localized oil pooling therapy for cervical pain)
- Katee Vasti (localized oil therapy for lower back pain)
These therapies helped reduce nerve compression, improve lubrication around the joints, and ease muscular stiffness.
The patient also began sleeping better, as nighttime numbness and radiating pain reduced gradually.
Digestive complaints such as constipation, bloating, and post-meal acidity showed visible improvement due to bowel-regulating therapies and dietary correction.
Day 7–10: Panchakarma Therapies for Nerve and Spine Recovery
During this phase, deeper Panchakarma treatments were introduced to address spinal degeneration, nerve irritation, inflammation, and Vata imbalance.
Vaitharana Vasthi Phase (16/10/2024 – 20/10/2024)
The patient underwent:
- Vaitharana Vasthi (medicated enema therapy for severe pain, inflammation, and nerve-related disorders)
This therapy played a major role in reducing radiating pain, inflammation, stiffness, and neurological discomfort affecting both upper and lower limbs.
The patient also underwent:
- Musthadi Rajayapana Vasthi (nourishing medicated enema therapy for neurological strength and tissue rejuvenation)
This nourishing therapy helped support spinal tissues, improve nerve function, and restore overall body strength.
During this period, the patient experienced:
- Better flexibility
- Reduced stiffness
- Improved posture
- Easier walking movements
- Improved energy levels
The pain continued to reduce steadily, and mobility improved considerably.
Day 11–13: Emotional Healing and Functional Recovery
As the physical symptoms improved, the patient also experienced significant emotional relief.
Long-standing mental stress caused by chronic pain and lifestyle limitations had affected his emotional well-being. Daily meditation, pranayama, yoga, and insightful discussions with Guruji helped calm the mind and improve emotional stability.
The patient reported:
- Better mental clarity
- Improved sleep quality
- Reduced daytime fatigue
- Greater emotional calmness
- Reduced anxiety related to pain
The integrated healing approach helped the patient feel physically lighter, mentally calmer, and emotionally more balanced.
Day 14: Significant Reduction in Pain and Improved Quality of Life
Toward the end of the treatment period, the patient’s pain intensity had reduced dramatically from 6/10 to 1/10.
The remaining discomfort was only mild aching pain, with major relief from:
- Radiating nerve pain
- Neck stiffness
- Lower back pain
- Numbness
- Digestive discomfort
- Sleep disturbances
The patient experienced:
- Improved spinal flexibility
- Better mobility
- Reduced inflammation
- Improved posture and gait
- Better digestion and bowel movements
- Improved sleep and overall energy
At discharge, the patient was advised to continue yoga, pranayama, dietary discipline, and follow-up care. A review treatment was recommended after six months to one year for long-term spinal maintenance and prevention of recurrence.
Conclusion
This case highlights how an integrated Ayurvedic approach can support recovery in complex spinal conditions involving cervical disc bulges, lumbar degeneration, nerve compression, digestive imbalance, and chronic pain.
Through Panchakarma therapies, personalised medicines, yoga, pranayama, diet correction, and emotional guidance, the patient experienced significant relief in pain, flexibility, mobility, digestion, sleep quality, and emotional well-being within approximately 14 days.
